I am at 99.5% I have...
57 missions
94 areas
7 gang hideouts cleared
5 rare weapons
5 job locations
13 apartments
12 oufits
18 strangers
completed all challenges
20 bounty locations
and positive scores in all gambling games
how an i missing that 0.5 % ???

Did you make a profit in all mini-games? : Poker, Blackjack, Horseshoes , Five Finger Fillet, Liar's Dice, Arm Wrestling.
Rentable houses don't count.
I think you missed one of the Stranger Missions. There are really a total of 19 Stranger Missions, but I Know You doesn't count towards 100% completion. The one people seem to miss a lot is Aztec Gold found in Sidewinder Gulch of Mexico. To check completed Stranger Missions go to Stats > Strangers.
